Purpose
Use this two-minute pre-shift check to confirm that the person, plan, device and response arrangements are ready before lone work begins. Adapt it to the role, risk assessment and local escalation procedure.
Shift details
Record the worker, date and start time, work location or route, expected finish time and agreed check-in points.
Ready to work?
- Confirm the visit, route or task risks and any changes since the last assessment.
- Confirm check-in times, the missed check-in route and the named escalation contact.
- Check the phone, app or alarm is charged, signed in, carried correctly and within reach.
- Check signal, location permissions and data connection where required.
- Confirm how to start and cancel an alert and what the responder will do.
- Confirm contact, vehicle and location information is current.
- Raise any new person, place, animal, access, travel or timing risk.
- Confirm authority to pause, leave, rearrange or call emergency services.
If any answer is no
Do not treat the checklist as permission to continue. Record the issue, tell the named supervisor and agree a safer plan before the work proceeds.
